Cardiac rehabilitation insurance coverage:

Most insurance carriers provide you with coverage for cardiac rehabilitation services that are directed and supervised by licensed physicians. However, you must be referred by your physician using one of the following *diagnoses:

  1. Post Myocardial infarction (heart attack)
  2. Post Coronary By-pass Graft
  3. Post Percutaneous Transluminal Angioplasty (PCI) or Stent
  4. Current Stable Angina
  5. Post Heart Valve Repair or Replacement
  6. Post Heart-Lung Transplant
  7. Current Stable Chronic Heart Failure
  8. Symptomatic Peripheral Artery Disease

*Diagnostic occurrence date for Myocardial infarction should be within 12 months of entry to our Cardiac Rehabilitation program
