Patient Portals

The Stamford Health and Stamford Health Medical Group Patient Portals help engage patients and families in their care by providing a secure and confidential web-based tool for accessing your health information. Please note you may have multiple portals depending on where you were treated and their features may vary. Learn more about each patient portal's offerings and how to enroll below.

As of April 5, 2021, Stamford Health will enhance the Stamford Health Medical Group and Hospital patient portals to comply with the recent 21st Century Cures Act. The Cures Act ensures patients have access to their electronic medical records via the patient portal without delay. All completed provider notes, laboratory tests and diagnostic tests will be available on the portal for both historical and recent visits, this may also include previously unavailable sensitive results such as HIV, STD, genetic testing, etc. Please make sure only appropriate individuals have access to your portal through the proxy access. If you have any questions about accessing the patient portal, please contact the Health Information Management Department at 203.276.7034.

Important Notice:
After April 5, 2021 test results will be made available without delay. You may see your results or care findings before your healthcare provider has reviewed them, please allow time for your healthcare provider to review the results. If you have a medical concern or question, please contact your healthcare provider directly.

How to Enroll into Stamford Hospital Patient Portal

  • Your portal enrollment card and medical record number are required to enroll in the Stamford Hospital Patient Portal.
  • If you already have your portal enrollment card and your medical record number, you can self-enroll.
  • If you do not have your portal enrollment card and medical record number, contact the Medical Records Department at (203) 276-7034.

Stamford Virtual Health Kiosk

Enroll into the Stamford Virtual Health Kiosk Patient Portal

In order to enroll, you'll need your First Name, Last Name, Date of Birth and Pin. Your pin will be provided to you at your visit.

Once you have entered your information on the home screen, you can establish a username and password, and complete your contact information.
