Stamford Hospital Receives Get With the Guidelines Silver Quality Achievement Award

Published: July 22, 2014

Stamford Hospital received the Get With The Guidelines®–Heart Failure Silver Quality Achievement Award for implementing specific quality improvement measures outlined by the American Heart Association (AHA)/American College of Cardiology Foundation’s secondary prevention guidelines for heart failure patients.

Hospitals receiving the Get With The Guidelines Silver Performance Award have reached an aggressive goal of treating heart failure patients with 85% or higher compliance to AHA core standard levels of care for 12 consecutive months.

According to the American Heart Association, about 5.1 million people suffer from heart failure. Each year, 670,000 new heart failure cases are diagnosed and more than 275,000 people will die of the disease. However, many heart failure patients can lead a full life when their condition is managed with proper medications and devices together with healthy lifestyle changes. Get With The Guidelines–Heart Failure is a quality improvement program that helps hospital teams provide the most up-to-date, research-based guidelines with the goal of speeding recovery and reducing hospital readmissions for heart failure patients.

“Stamford Hospital’s Heart & Vascular Institute is the only full-service cardiovascular health program in lower Fairfield County and we are pleased to again be recognized for providing our community with a program that meets the AHA’s rigorous standards,” said Steven F. Horowitz, M.D., F.A.C.C, the Medical Director of Planetree and Cardiac Care Management, Cardiology Chief Emeritus at Stamford Hospital. “Our entire team is dedicated to making our care and long-term outcomes for heart failure patients among the best in the country, and implementing the AHA’s Get With The Guidelines–Heart Failure program has helped us accomplish this goal.”

Stamford Hospital’s Heart & Vascular Institute is a full-service cardiovascular facility with specialties in cardiology, cardiac surgery, vascular services, screening and diagnostics and cardiac rehabilitation. A collaborative team also provides cardiovascular disease prevention and wellness, with education programs designed to teach patients how to keep their cardiovascular systems healthy.
