Anticoagulation

Patients prescribed warfarin (Coumadin®) therapy often require assistance in managing food and drug interactions, genetic predispositions, and the maintenance of therapeutic INR values (International Normalized Ratio). Sub-therapeutic (below the normal result) or supra-therapeutic (more than the normal result) INR values may result in complications placing patients at risk for clotting or bleeding.

Stamford Hospital’s Anticoagulation Service Center offers a “physician referral only” program designed to help manage these complex dose-response relationships. Working closely with a patient’s physician, our expert clinical pharmacists adjust warfarin doses as necessary in order to achieve INR goals set by the patient’s physician.
