Stamford Health Medical Group Awarded National Recognition for Patient-Centered Care

Patient satisfaction, long-term outcomes benefit from NCQA Patient-Centered Medical Home™ Standards, emphasized by enhanced care through the patient-clinician partnership

The Stamford Health Medical Group received Level Three Patient-Centered Medical Home (PCMH) recognition by the National Committee for Quality Assurance (NCQA) for their primary care practices, which include offices in Stamford, Greenwich, Norwalk, Darien, New Canaan and Wilton. This is the highest PCMH distinction awarded to medical practices by the NCQA for the use of evidence-based, patient-centered processes focusing on highly coordinated care and long‐term, participative relationships between clinicians and patients.

“This unique care model encourages every patient to actively participate in managing their health and the care they receive – they are truly the most important member of the team,” said Rod Acosta, MD, President and CEO, Stamford Health Medical Group. “Each patient’s care is overseen by clinician-led teams to coordinate treatment across the entire healthcare system.”

The NCQA Patient-Centered Medical Home is a primary care model that combines teamwork and information technology to improve outcomes, enhance the patient experience and reduce costs. Medical homes foster ongoing partnerships between patients and their clinicians, rather than approaching care as a series of episodic visits.

The Medical Home team works closely with the patient to ensure their individual needs are met by understanding their life situation, health goals and preferences; offering extended office hours and timely clinical advice by phone or through an online patient portal; encouraging family members and/or other members of the patient’s support team to participate in clinical visits; working collaboratively with all of the patient’s healthcare providers; and engaging patients to be involved as a member of their own health team.

To earn recognition, which is valid for three years, Stamford Health Medical Group Primary Care demonstrated the ability to meet the program’s key elements, embodying characteristics of the medical home. NCQA standards align with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.

NCQA is a private, non-profit organization dedicated to improving healthcare quality. NCQA accredits and certifies a wide range of health care organizations. It also recognizes clinicians and practices in key areas of performance.
